The Election Commission of India (ECI) has started the Special Summary Revision (SIR) 2025 process in West Bengal. During this period, citizens can verify, update, or include their names in the voter list. To complete this process, electors need to fill-up two important forms —

Annexure-III (Enumeration Form)
Annexure-IV (Declaration Form).

What is the SIR Form in West Bengal?

SIR stands for Special Summary Revision is a yearly process conducted by the Election Commission to update the electoral roll
(voter list)

During SIR, citizens can:
  • Include their names in the voter list (Form 6)
  • Correct or update details like address, photo, or age (Form 8)
  • Verify existing voter details
The SIR Form is used by BLOs (Booth Level Officers) to collect and verify the information of each elector.

🔹 Annexure-III: Enumeration Form (SIR Form) -Enumeration SIR Form

This is the main form used during the SIR verification process. The Enumeration Form includes pre-printed details like:
  • Elector’s Name, EPIC Number, Address
  • Part Number, Serial Number, Assembly Constituency Name
  • Old Photo and space for a new photo
  • QR Code
The voter or their family member must verify all information and fill in missing details.

Details to be filled by Elector:
  • Date of Birth (DD/MM/YYYY)
  • Aadhaar Number (Optional)
  • Mobile Number
  • Father’s/Guardian’s Name and EPIC (if available)
  • Mother’s Name and EPIC (if available)
  • Spouse’s Name and EPIC (if applicable)
Previous SIR Details Section:

Electors need to provide:
  • Details of their name or a relative’s name in the previous SIR Electoral Roll
  • District, State, AC Name, AC Number, Part Number, and Serial Number

This helps the Election Commission verify old voter records.


Important Declarations (to be signed by voter/family member):
  1. The elector has not acquired citizenship of any other country.
  2. The elector’s name is not included in any other constituency.
  3. Any false declaration is punishable under Section 31 of the Representation of the People Act, 1950.

Finally, the BLO (Booth Level Officer) verifies the details and signs the form.

🔹 Documents Required for SIR Form Verification

Depending on your date of birth, the following documents may be needed:

If born before 01.07.1987:
  • Any identity card or certificate issued by a government authority before 1987
  • Birth certificate, passport, educational certificate, or residence proof
If born between 01.07.1987 and 02.12.2004:
  • Self and parents’ documents showing date/place of birth (e.g. Birth Certificate, Matric Certificate, Passport)
If born after 02.12.2004:
  • Proof of birth and Indian citizenship of parents at the time of birth
Accepted Documents (Indicative List):
  1. Government/PSU Identity card or Pension Card
  2. Birth Certificate
  3. Educational or Matriculation Certificate
  4. Passport
  5. Permanent Residence Certificate
  6. Caste Certificate (SC/ST/OBC)
  7. NRC Certificate (where applicable)
  8. Family Register or Land/House Allotment Certificate

🔹 Annexure-IV: Declaration Form - SIR Declaration Fill Up

The Declaration Form (Annexure-IV) is submitted along with Form 6 or Form 8.
It confirms that the applicant:
  • Has not acquired foreign citizenship
  • Is applying for inclusion or shifting in the electoral roll
  • Is not enrolled in any other Assembly or Parliamentary Constituency
This form also includes details of parents, spouse, and previous voter record (if any).
The voter signs the declaration, and the BLO verifies it.

  • Collect Form from your local BLO or download the SIR Form PDF.
  • Fill in your personal details (name, DOB, Aadhaar, etc.).
  • Paste a recent passport-size photo.
